QME: This provider was approved as a panel Qualified Medical Evaluator by the Division of Workers Compensation, the State of California until he voluntarily let his appointment expire in 2017. Fees and procedures for a qualified psychiatric or psychological evaluation are set by state law.

AME: When all parties AGREE in writing, this provider can be retained to perform an Agreed Medical Evaluation for a Workers' Compensation Claim with fees set in advance as part of the agreement and retainer expected prior to the commencement of the evaluation. This evaluation will meet the criteria for a psychiatric or psychological evaluation set by the Division of Workers' Compensation Medical Unit.
